Main Dataset: Medicaid Provider Spending
opendata.hhs.gov — Medicaid Provider Spending
This is the main dataset we use. It comes from the T-MSIS system (Transformed Medicaid Statistical Information System), which is how every state reports their Medicaid spending to the federal government. We filter exclusively for Nevada data. It includes every payment made to every provider, broken down by the type of service, the month it happened, and the provider's NPI number. It covers 2018 through 2024.

Other Data Sources We Use
NPPES NPI Registry
npiregistry.cms.hhs.gov
The official registry of every healthcare provider's NPI number, name, address, and specialty. We use this to look up provider names and match them to their Medicaid payments. The raw spending data only contains NPI numbers — names come from this registry.
OIG LEIE Exclusion List
oig.hhs.gov/exclusions
A list of individuals and organizations that have been banned from participating in Medicaid. If someone on this list is still receiving Medicaid payments, that is a federal crime.
Nevada Attorney General — Medicaid Fraud Cases

Press releases from the Nevada Attorney General about Medicaid fraud cases that have been investigated and prosecuted in the state.
Nevada Secretary of State — Campaign Finance

Campaign contribution records from Nevada state and local elections. We cross-reference Medicaid provider business registrations and their corporate officers against campaign finance contributor records to identify which providers and their executives contribute to Nevada politicians.
Nevada Secretary of State — Business Entity Search (Aurora)

Corporation and LLC filings from the Nevada Secretary of State. We use this to verify provider business entities, identify registered agents, and look up corporate officers for contribution matching.
FEC Candidate Master File
fec.gov
Federal Election Commission candidate registration data. We use this to list all Nevada candidates who have run for U.S. Senate and U.S. House from 2020 onward, including their party affiliation, district, and incumbent/challenger status. Covers all election cycles from 1980 through 2026.
FEC Individual Contributions
fec.gov
Individual contribution records filed with the Federal Election Commission. We filter for Nevada contributors and cross-reference them against Medicaid provider corporations and their officers. Covers election cycles from 2018 through 2026, totaling 2.9 million Nevada records.
FEC Committee Contributions
fec.gov
Committee-to-candidate contribution records (Schedule B) from FEC filings. Shows which PACs, party committees, and other political committees contributed to Nevada federal candidates. 103,639 records from 5,531 donor committees to 203 NV candidates, covering all election cycles from 1980 through 2026.
FEC PAC & Party Summary

Financial summary data for all Political Action Committees (PACs), Super PACs, and party committees registered with the FEC. Includes total receipts, disbursements, independent expenditures, and cash on hand. Covers all election cycles from 1996 through 2026.
FEC Operating Expenditures

Operating expenditure records from FEC filings showing how campaign committees spend money in Nevada. 187,383 expenditure records to NV recipients from 3,192 committees, covering election cycles from 2004 through 2026.
Important Notes
This data shows payments, not fraud. Just because a provider received a large amount of money does not mean they did anything wrong. Many providers earn high payments because they serve more patients, work in expensive specialties, or practice in underserved areas of Nevada. The reports on this site are designed to help people understand where taxpayer money goes and to flag unusual patterns that may deserve a closer look — not to accuse anyone of wrongdoing.
All data on this site is limited exclusively to the state of Nevada. No data from any other state is included. The spending figures represent payments made through Nevada's Medicaid program, as reported to the federal government through the T-MSIS system.
